Welcome to Smile Creations Dental Practice.  We thank you for allowing us to serve you with your health care needs.  We are dedicated to providing you with individual attention in order to create a beautiful and healthy smile.  As a new patient, Dr. Tsui will go over a comprehensive exam including dental radiographs (X-rays), and a visual exam.

Now you may download and print out your dental forms at your own convenience at home or work and bring them into the dental office before your first visit.  It saves time and it's easy.
