• Membership plans are not insurance and is not a qualified health plan under the Affordable Care Act.
• There is a $149 one time activation fee.
• Payment for any treatment that is not included in the membership plan will be collected from the patient or authorized parent/guardian.
• Benefits of the membership plan may not be combined with insurance, other offers, or discounts.
• Membership plans do not cover procedures completed by specialists outside our practice.

Child Membership Plan  Only $34/month or $408 per year
